I've served as a Private Caregiver, caring for many families and their loved ones; I've recognize a significant gap in the Healthcare industry: the tendency for care to become "Standardized" and Impersonal. I have cared for clients in the Greystone/ Shelby County, Birmingham & Vestavia Hills Communities, worked in Mountain Brook as a Cook to help feed my dreams of starting this business! 2 hr. Service Minimum

Most Home Health Companies require a 4 hr. Minimum but Not US! We understand that every client is different and has their own special needs. This is for the client who need someone to accompany them to a Hair Appointment, Grocery Store run or Specific Errand that can be accomplished in the two hour window!

In our current Home Health Care System; if a Client has shown "Progressive Improvement" they are "Dismissed" when they meet their "Care Goals" and No longer requires Skilled Care. That's the difference in Uniquely Living and the others; We stay around to provide Continuum Care like Medication Reminder, Measure and Record Daily Vital Signs. Proactively Addressing The Need as it Arise not when it's Discovered!

HOW IT WORKS

* Give US a call 866-530-4068 * Our Client Care Coordinator will come out and Complete the Home Care Assessment; evaluating the client's Medical and Personal requirements to find the ideal match with one of our Trained & Professional Caregivers * Sign all forms in the Personal Care Agreement packet, giving us permission/ Consent to care for your loved one, which includes: The Financial Responsible Party Agreement, Severance Pay Agreement, this is an upfront deposit of 2 weeks of the estimated care cost. The Personal Care Agreement page itself! * In the coming days, Typically 2-3 days we will finalize the caregiver matching process based on the New Client Assessment * We will then give you a call to inform you of the wonderful caregiver that you have been matched with, who will then come out and began Providing Personalized Tailored Care! Collaboration, Connecting and Actively Listening to the client, is how we customize a plan to care, based on their Unique lifestyle, medical history and personal goals!
